Prognosis of Acute Middle Cerebral Artery Infarction with Intractable Hiccup

Abstract
Background: The intractable hiccups (IH) are defined as hiccup persisting for more than 48 hours. In general, the IH are associated with dysphagia, loss of airway control, aspiration pneumonia and weight loss. There are no reports that prognostic significance of IH in patients with acute middle cerebral artery (MCA) infarction. This study was designed to investigate whether the IH are a poor prognostic factor in MCA infarction.

Methods: We identified 16 acute MCA infarction patients with IH by medical record, telephone interview and brain MRI and/or CT between 1998 and 2003. Fifteen patients without IHs were included as control group. We excluded any patients who might have had any other causes of hiccup. Using National Institutes of Health Stroke Scale (NIHSS) score, Barthel index (BI) and modified Rankin scale (mRS), we compared two groups on admission, 3 days, 7 days and 3 months. We analyzed survival period by Kaplan-Meier method.

Results: All patients with IHs were men. The mean onset time of IHs was 4.1 days (range, 2¡­9 days). The mean duration of IHs was 5.9 days (range, 3¡­11 days). There were no differences in NIHSS score, BI and mRS between IH group and control group. There was no difference in survival between two groups.

Conclusion: These results show that occurrence of IH after acute MCA infarction are frequent in men and does not have prognostic significance.
